This GP Practice along with others in North East Essex provides information into a risk stratification tool.   

 

The tool uses automated decision making to identify patients who are at risk of adverse outcomes such as unplanned hospital admissions and who may benefit from additional preventative support.   

Your identifiable information will only be available to your GP. 

 

If you do not wish your information to be used or would like further information please contact the Practice.  If you have already opted out of Summary Care Record your data will not be uploaded into the tool.
